Legende

  • Ak: Antikörper
  • tTG-Ak: Transglutaminase-Antikörper
  • HLA: humanes Leukozytenantigen
  • Marsh-Stadium: histologische Klassifikation der Zöliakie:
    • Marsh 2: ≥ 40 intraepitheliale Lymphozyten / 100 Enterozyten (Zellen der Darmschleimhaut) + Kryptenhyperplasie
    • Marsh 3: ≥ 40 intraepitheliale Lymphozyten / 100 Enterozyten + Kryptenhyperplasie + Zottenatrophie

Marsh-Klassifikation [2]

  Typ 0 Typ 1 Typ 2
Intraepitheliale Lymphozyten
(Schwellenwert > 40 IEL/100)
< 40/100 > 40/100  > 40/100
Krypten normal normal hypertroph
Zotten normal normal  normal
Diagnose
  • nur bei bekannter Zöliakie und histologischer
    Vollremission (vollständige Rückbildung)
  • vollkommen normale Schleimhaut
  • infiltrativer Typ
  • potentielle Zöliakie
  • muss weiter abgeklärt werden, d.h. Serologie, evtl. Histologie (feingewebliche Untersuchung) nach Glutenbelastung
  • hyperplastischer Typ
  • meist "patchy lesions" bei der Endoskopie in Stufen auch Typ 1 oder Typ 3 Läsionen

Typ 3a Typ 3b Typ 3c Typ 4
> 40/100 > 40/100 > 40/100 < 40/100
hyperplastisch hyperplastisch hyperplastisch normal
leichte Atrophie subtotale Atrophie totale Atrophie totale Atrophie
  • typische histologische Bild einer Zöliakie
  • destruktiver Typ
  • 3b – hochgradige Zottenverplumpung (nur kleine Zottenreste erkennbar)
  • 3c – flache Schleimhaut
  • für Zöliakie diagnostisches Stadium !!
  • extrem selten, früher wahrscheinlich Folge einer hochgradigen Mangelernährung bei Patienten mit "übersehener Zöliakie"
  • hypo-plastischer Typ
  • keine nenneswerte Infiltration der Lamina propria (eine unter Epithelien anzutreffende Schicht aus Bindegewebe)
